OUTSTANDING YOUNG ALUMNI AWARD NOMINATION FORM

The Outstanding Young Alumni Award recognizes early-career Illinois graduates (DVM, PhD, and MS degree holders) who have excelled early in their respective fields and who embody the Illini spirit of “labor and learning,” through creativity and entrepreneurship in the field of veterinary medicine, regardless of where they reside.

Any individual or organization interested in submitting a nomination of an alumna/alumnus of the University of Illinois College of Veterinary Medicine is welcome to do so. Self-nominations are also accepted. Nominees should be young alumni, graduating within the last 10 years. The Selection Committee will select one recipient each year. Nominations will remain active for three years.

Completed forms with supportive materials should be submitted to the Office of Advancement no later than June 30, 2023. Self-nominations are welcome. Recipients will be notified in July so travel arrangements and campus plans can be coordinated.

Eligibility and Criteria

Any graduate in possession of a DVM, PhD, or MS degree from the University of Illinois College of Veterinary Medicine at Urbana-Champaign within the last ten years is eligible for nomination.

The nominee’s contributions to the profession may have been made in any of the following areas:

  • practice – clinical, diagnostic, public health, regulatory, military
  • shelters or humane societies
  • teaching
  • research

  • Please write one letter of support in addition to this form. Explain how the nominee has embodied the Illini spirit of “labor and learning,” through creativity and entrepreneurship in the field of veterinary medicine. If self-nominating, the letter of support must be from a third party.
