- Start your form from Vetstar to have all of the patient, owner, and RDVM information automatically entered into them. Click here for instructions on release/referral/surgery forms, and here for SOAP forms. They are also available for download at: http://www.cvm.uiuc.edu/vetstar.
- Macros are embedded in the VTH forms for running the Spell Checker and saving the file for upload to Vetstar. On VTH computers, the macros should be automatically enabled. If prompted, click to 'Enable Macros' button when opening your form. See http://www.cvm.uiuc.edu/vetstar/VTHForms/FAQ.html for more information on macro settings.
- Please be sure to complete the patient name, medical record number, and visit number and/or date. This information is needed for importing the report into the correct visit in the Vetstar record.
- Use the Tab or right arrow key to move from one field to the next (Shift/Tab or left arrow to back up). Do not use the Enter key; this will add extra empty lines to your report.
- To use the keyboard to access the items in the drop down lists: Use the Alt key + up or down arrows to select, then hit the Enter key.
- Most of the fields are set to automatically capitalize the first word (seen after you tab to the next field).
- You may enter the dates in any format; they will automatically change to the long format when you leave the field.
- Some of the forms have reference fields - they display information that you have entered into the form fields in other parts of the document. For example, the patient name is copied from the 'Patients Name' field at the top of the report to another place near the bottom of the form. These fields will be updated when you print or 'Print Preview' the document.
- Don't forget to check your spelling! To spell check your report, click the Spell Check icon
at the top of the document on the toolbar. There is also a Spell Check option under the Tools menu and the custom 'VTH Forms' menu at the top of the form.
- Print an extra copy to keep in the record. Please sign or obtain a signature for each doctor listed below a signature line. If a doctor will not be cosigning a form, select a blank line for the second doctor field. If a name is selected and the doctor did not sign above his or her name, Medical Records will return the form to obtain a signature there.
- If the letter is to be faxed, place it into the SAC mail slot labeled "RDVM Letters" or the fax tray in the LAC Medical Records office. The night student worker will fax it out for you. Please include the area code with the FAX number when you enter it on the form.
- If you need to save the document for review by the doctor(s) in charge of your case, there is a network directory on the CVM-AD to place your file so they can see it:
- In the future, a shortcut will appear on VTH computer desktops labeled 'VTH Preliminary Reports'. Save your file there. All students, faculty and staff will be able to read and write files to this directory. Please do NOT save other files to this directory.
- This is for TEMPORARY file storage only. The purpose of this directory is to have a shared space that everyone can access for reviewing preliminary reports. Old files will be routinely deleted from this location. If you would like to save your reports in Word format for your own future reference, you must do so in a different location. If you save the document for import to Vetstar, you may access them from the Vetstar record.
- If you do not have a short cut to this directory on your computer's desktop, you may access it by typing the following into Start->Run:
\\Felix\VetstarUpload\PrelimReports (or you may create a 'Network Place' to this directory).- If you need help accessing or creating a shortcut to this directory, please contact VTH desktop support or vetstar-help@cvm.uiuc.edu.
- To save the document for import into the Vetstar record (do this on FINAL reports only):
- The macro converts your document to a simple text format and saves the text file in a network directory for upload to Vetstar.
- Save a backup copy of your document first, if you would like to save it in Word format. You will be prompted to do this by the macro. If you wish to save it, please do so before the macro runs! The document will close automatically when the macro has finished. The report will be uploaded to Vetstar in the near future (there may be a lag time of a week or so after it has been saved), and will be permanently stored in your patient's Vetstar record. - Make sure that you are logged into the CVM-AD (you log into the CVM-AD when you log into Windows on the computers in the VTH).
- Print your report before running the macro. You will be prompted to print when the macro runs. However, if the printer is jammed and your job does not print, you may lose your report, since the macro closes the file after it is finished.
- Run the save macro by clicking on the 'Save for Vetstar' icon
on the document's toolbar, then follow the prompts. You may also run this by choosing the 'Save for Vetstar' option under the custom 'VTH Forms' menu at the top of the form. When the macro is finished, it will report that the file has been converted and saved, and the file will close. See http://www.cvm.uiuc.edu/vetstar/VTHForms/SaveForVetstar.html for more instructions.
