Rotation requirements:

In order to take an OC Primary Care Rotation at a private practice, the practitioner must submit a Willingness to Participate Agreement – Word or Willingness to Participate Agreement – PDF. Both the practitioner and student must review the Learning Objectives in order to be informed of expectations while completing this rotation.

All Students must submit the OC Rotation Scheduling Form at least 6 weeks before leaving for the rotation to the ASA Office.

During the rotation, the student must complete a Primary Care Activity Checklist (not a caselog).

At the end of each 2-week block, the practitioner must submit a Student Performance Evaluation and the signed Checklist to Karen Stahl in the VCM Office.

Database of OC Primary Care sites:

You can run a Sites Report in E*Value to see where students have taken OC Primary Care rotations. Practices do NOT need to be in the database, or approved by the Curriculum Committee for you to go there, as long as the above requirements are met.
