Registration is open to veterinarians. Maximum registration for course it is 20.

The registration fee includes handout materials, continental breakfast, lunch, breaks, laboratories, and course materials. Payment must be enclosed to guarantee registration. The prices listed below are per person. The early registration deadline is Sunday, February 6, 2011.

On or Before Sunday, February 6, 2011 - $750
After Sunday, February 6, 2011 - $800

Course fees are payable by check or money order to the University of Illinois or by Visa, Master Card, Discover, or American Express and are due at the time of registration. Early registration is recommended.

Registration can be placed online by visiting vetmed.illinois.edu/ope/surgery/ and a copy of the printed registration form is also available to download.

Continuing Education
Veterinarians can obtain up to 5.75 CEUs for attendance at the course. Participants will receive continuing education verification at the end of the course.

The course will be held at Hawthorn Animal Hospital, which is located at 5 Cougar Dr., Glen Carbon, Ill.

Hotel Accommodations
Course attendees are responsible for making their own hotel reservations. The hotel nearest to the course location is Country Hearth, 1013 Plummer Dr., Edwardville, Ill., 618 656 7829.

Cancellation Policy
Full refunds, less a $150 cancellation fee, will be given if written notification is received by Sunday, February 6, 2011. Written cancellation should be sent to ope@vetmed.illinois.edu or to the same mailing address to which registrations are sent. Registrations are transferable. After February 6, 2011, refunds will not be made. We reserve the right to cancel programs and to change speakers if necessary. In the event of course cancellation, all registrants will receive a full refund. The University is not responsible for travel expenses in the event of a cancellation or a date change.

Course Check-In
Course materials will be available on the day of the course at Hawthorn Animal Hospital, which is located at 5 Cougar Dr., Glen Carbon, Ill.
