Registration
Registration is open to veterinarians and their technicians. Technicians must register with a veterinarian; veterinarians can register as an individual. Maximum registration is 27 veterinarians and 9 technicians.

The registration fee covers the cost of handout materials and meals and breaks listed on the schedule. The prices listed below are per person.

The early registration deadlines for this course is Sunday, May 23, 2010.

Course fees are payable by check or money order to the University of Illinois or by Visa, MasterCard, Discover, or American Express and are due at the time of registration. Early registration is recommended.

Continuing Education
Veterinarians and certified veterinary technicians can obtain 7.75 continuing education units for attendance at the course. Participants will receive continuing education verification at the end of the course.

Location
The course will be held on the University of Illinois College of Veterinary Medicine campus, which is located on S. Lincoln Ave., in Urbana, Ill. For maps and directions visit vetmed.illinois.edu/ope/.

All lectures will be held in room 100 of the Large Animal Clinic, which is located at 1102 W. Hazelwood Dr., Urbana, Ill. Entrance to this facility is through the courtyard area between the Large and Small Animal Clinics. The hands-on laboratory will be held in one of the surgery suites within the Small Animal Clinic.

Course Check-In
Course materials will be available on the day of the course just inside the entrance located off of the courtyard area between the Large and Small Animal Clinics.

Cancellation Policy
Full refunds, less a 10 percent cancellation fee, will be given if written notification is received by the early registration deadline for each course. Written cancellation should be sent to ope@vetmed.illinois.edu or to the same mailing address to which registrations are sent. Registrations are transferable. After the early registration deadline, refunds will not be made.

We reserve the right to cancel programs and to change speakers if necessary. In the event of course cancellation, all registrants will receive a full refund. The University is not responsible for travel expenses in the event of a cancellation.

Transportation
The University of Illinois is located near Interstates 57, 72, and 74. The community is also accessible by train via Amtrak and airplane through Willard Airport (CMI) at Savoy. The course will be held at the University of Illinois Large Animal Clinic, which is located at 1102 W. Hazelwood Dr., Urbana. For maps and directions, visit vetmed.illinois.edu/ope/.

Hotel Accommodations
Course attendees are responsible for making their own hotel reservations. Room blocks will be held through Thursday, May 6, 2010. The below listed rates can be obtained by requesting the “Vet Med Minimally Invasive Surgery” room block. All prices listed are not inclusive of taxes.

Parking
Course attendees should park in lot F27 and follow the signs to the check-in. The entrance to this lot is off of S. Lincoln Ave. Course attendees should NOT park in the Large or Small Animal Clinic parking lots. These lots are reserved for weekend clients and staff.

Messages
In an emergency, messages can be left for course participants at 217/333-2907 during course hours. As a courtesy to other registrants, we request that all mobile phone ringers be muted for the duration of the program.
