Registration
Registration is open to veterinarians; maximum registration is 20.

The registration fee includes handout materials, continental breakfast, lunch, breaks, laboratories, TightRope CCL video, and a surgical guide. Payment must be enclosed to guarantee registration. The prices listed below are per person. The early registration deadline is Sunday, November 8, 2009.

On or Before Sunday, November 8, 2009 - $750
After Sunday, November 8, 2009 - $800

Course fees are payable by check or money order to the University of Illinois or by Visa, MasterCard, Discover, or American Express and are due at the time of registration. Early registration is recommended.

Continuing Education
Veterinarians can obtain up to 5.75 CEUs for attendance at the course. Participants will receive continuing education verification at the end of the course.

Location
The course will be held on the University of Illinois at Chicago campus in the Biologic Resource Laboratory, which is located at 1840 W. Taylor St., Chicago.

A campus map with important landmarks marked will be provided with registration confirmation.

Hotel Accommodations
Course attendees are responsible for making their own hotel reservations. Room blocks will be held through Thursday, October 22, 2009. The below listed rates can be obtained by requesting the “Minimally Invasive Cranial Cruciate Ligament Surgery” room block. All prices listed are not inclusive of taxes.

Hotel room cancellations must be received 24 hours prior to arrival or one night’s room and tax will be assessed by the hotel.

Cancellation Policy
Full refunds, less a $150 cancellation fee, will be given if written notification is received by Sunday, November 8, 2009. Written cancellation should be sent to the Office of Public Engagementor to the same mailing address to which registrations are sent. Registrations are transferable. After November 8, 2009, refunds will not be made.

We reserve the right to cancel programs and to change speakers if necessary. In the event of course cancellation, all registrants will receive a full refund. The University is not responsible for travel expenses in the event of a cancellation.

Course Check-In
Course materials will be available on the day of the course just inside the entrance to the Biologic Resources Laboratory, which is located at 1840 W. Taylor St., Chicago.

Parking
Course attendees should park in the Wood Street Parking Structure, which is located at 1100 S. Wood St., Chicago.

Upon entering the parking structure pull a parking ticket and take it with you to course check-in, where you will be provided with a sticker to validate your parking. It is important to note that the University cannot be held responsible for parking tickets or tows incurred as a result of parking in an area other than that which was directed.
