Special Service Award

SPECIAL SERVICE AWARD NOMINATION FORM

The Special Service Award recognizes an outstanding individual or organization that has made significant contributions to the advancement of the veterinary profession and/or the University of Illinois College of Veterinary Medicine at Urbana-Champaign. This individual or organization can be a non-veterinarian or a veterinarian who holds a degree from an institution other than the University of Illinois College of Veterinary Medicine.

Any individual or organization interested in submitting a nomination is welcome to do so. The Selection Committee may select no more than one recipient each year. Nominations will remain active for three years.

Completed forms with supportive materials should be submitted to the Office of Advancement no later than June 20, 2023. Recipients will be notified shortly thereafter so travel arrangements and campus plans can be coordinated.

Eligibility and Criteria

Any organization, non-veterinarian or any veterinarian who holds a degree from an institution other than the University of Illinois College of Veterinary Medicine is eligible to be nominated for this award. Individuals may be nominated for this award posthumously. The nominee’s contributions to the profession may have been made in any of the following areas:

  • practice – clinical, diagnostic, public health, regulatory, military
  • shelters or humane societies
  • teaching
  • research
  • involvement in organized veterinary medicine
  • human-animal bond
  • mentoring of future veterinarians

  • Please include your letter of support in this field. Share the nominee’s service to the University of Illinois College of Veterinary Medicine or any other information that makes this nominee uniquely qualified for the Special Service Award.
